Job Summary
Case Manager positioned within Seneca Health Services’ behavioral health programs, responsible for evaluations, development and implementation of individualized treatment plans, and providing targeted case management and crisis services to individuals with behavioral health needs across outpatient and intensive treatment settings. Responsibilities include coordinating referrals and warm hand-offs to internal and external services, advocacy for client access to resources, crisis response planning, and maintaining client progress toward goals while upholding HIPAA and confidentiality. Requires a Bachelor's degree in a Human Service field, valid driver's license, and successful background check and drug screening.
