Job Summary
Case Manager (SE) coordinates and evaluates outcomes for defined patient populations using the nursing process; works Monday–Friday with on-call/holiday duties, collaborates with physicians, nurses, and multidisciplinary teams to ensure continuity of care and cost-effective outcomes; requires RN licensure and a BSN (MSN encouraged), with at least five years of experience in patient care and case management.
Required Qualifications
- Current registration to practice as a Registered Nurse in the applicable state (RN license)
- Minimum of five years of experience in the care of the assigned patient population
- BSN preferred; MSN encouraged
- Case Management certification encouraged
