Job Summary
Establish and maintain relationships with Transitional Aged Youth ages 16 to 21, implementing supervision, discipline, and community resource connections to support independent living skills and transition goals. Conduct weekly individual contacts to track progress, manage medications, and coordinate care with families, natural supports, and external agencies. Facilitate Child Family Team meetings, TDMs, and emancipation conferences while ensuring compliance with CCL regulations and THP Program Statement. Maintain accurate Medi-Cal reports, client databases, and case notes within required timelines. Cover shifts as needed and on an emergency basis.
Required Qualifications
- Bachelor's Degree in one of the following areas: Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare, an area that includes counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor
- Bachelor's Degree in one of the following areas: Human Services or Behavioral Science
- Two (2) years of experience in a public or private child welfare social services setting
- specific experience working with TAY 16 – 24 years of age
- CCL approval as an Excepted Case Manager prior to start date
- Experience working with the TAY population
- Knowledge of Microsoft Offices applications
- Demonstrated skills in case work, group work and case coordination
- Understanding of transformational care planning and targeted case management principle of service delivery
- Excellent interpersonal skills (verbal and written) with demonstrated leadership in facilitating and linking services among a diverse variety of mental health programs
- Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth and young adults
- Staff will be required to cover shifts as needed and on an emergency basis

- Two (2) years' experience working with at-risk youth or adolescent children in a residential group home setting
- Bi-Lingual in Spanish and English
