Job Summary
Behavioral Health Case Manager in San Francisco, CA to provide comprehensive case management for individuals with mental health and/or substance use disorders. Responsibilities include conducting comprehensive assessments, developing individualized care plans, coordinating therapy, medication management, and support groups, connecting clients with community resources (housing, food assistance, employment services, transportation), developing Comprehensive Treatment Plans, advocating for clients, providing crisis intervention, maintaining accurate documentation, collaborating with an interdisciplinary team, and communicating with residents’ families. RequiresBachelor's degree in social work, psychology, counseling or related field (Master's preferred), minimum 3 years of behavioral health case management experience, proficiency in EHR, CPR and First Aid certification, and mandatory pre-employment screenings.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling, or a related field (Master's preferred)
- Minimum of 3 years of experience in behavioral health case management
- Proficiency in electronic health records (EHR) systems
- Current CPR and First Aid Certification
- Successful completion of Pre-Employment Requirements including, a criminal background clearance, drug testing, and health screening, is mandatory prior to employment
- Experience working with diverse populations
- Knowledge of mental health and substance use disorders and treatment modalities
- Strong organizational and time-management skills
- Ability to communicate clearly with clients, staff, peers, supervisors and resources
- Understanding of State and Accreditation requirements: DHCS – Substance Use Disorders, JCAHO and regulatory agencies required
- Familiarity with 12-step model of recovery (AA, CA, NA, Alanon)
- Knowledge of community resources and social service systems
