Job Summary

Develop and implement an integrated, trauma-informed approach for transitional-aged homeless youth (ages 18-24) with co-occurring disorders. Establish rapport with diverse youth to support drop-in center services, including intake, needs assessment, crisis intervention, and resource linkage. Provide outreach to at-risk, runaway, or homeless individuals for intensive case management and advocacy across the TAY system of care. Monitor participant progress, maintain accurate records, and ensure data entry into approved databases. Coordinate informal and formal community resources while adhering to agency standards, confidentiality protocols, and HIPAA regulations. Work independently and as a team member to create innovative solutions that meet individualized participant needs.

Required Qualifications

  • Bachelor's degree preferred and/or minimum 2-years' experience working with high-risk adolescents
  • basis in Trauma-Informed care
  • dual diagnosis
  • cultural competency
  • Bi-lingual desirable
  • Completed Peer Specialist training preferred
  • working knowledge of trauma-informed care
  • dual diagnosis treatment
  • available to work weekends and evenings as assigned by supervisor and/or as needed
  • willing to and successfully complete background check
  • drug test
  • TB test
  • CPR and First Aid Certification
  • own, reliable transportation
  • ability to provide transport to youth as needed in the community
  • ability to lift up to 50 lbs
  • verify their eligibility to work in the United States

Desired Qualifications

  • Bi-lingual desirable
  • Completed Peer Specialist training preferred
