Job Summary
Conduct comprehensive psycho-social history assessments and family eligibility determinations for new residents. Develop individualized family service plans and life skills programs while facilitating quarterly parenting initiatives. Meet weekly with clients to review service plans, coordinate referrals for housing and employment, and implement orderly transitions to independent living with ongoing aftercare support. Lead weekly therapist team meetings and resident group sessions to align case management with therapeutic processes. Serve as a liaison to community agencies, manage client records, and participate in continuous quality improvement efforts. Available for evening hours and on-call visits as required to provide adequate services to the client population.
Required Qualifications
- Bachelor's degree in Social Work or related field
- Available for evening hours
- CPR certified
- Florida driver's license
- Excellent driving record
- Valid Florida driver license
- Ability to work irregular hours
- Occasional light lifting and carrying of under 15 pounds
- Occasional moderate carrying and lifting of 15 - 44 pounds
- Frequent use both hands and fingers
- Frequent extended periods of sitting
- Occasional periods of standing, kneeling and climbing
- Occasional ability to distinguish basic colors
- Occasional time spent working closely with others
- Ability to work effectively with employees, department heads, representatives of other agencies and the public
- Ability to multi-task, work independently and make responsible judgment calls
- Computer literate; Microsoft Word and Excel, email and internet usage
- Ability to understand the needs of the client and respond accordingly
- Ability to work as a team player and work collaboratively with other personnel and/or service provider or professionals
Desired Qualifications
- 1-3 years of experience preferred
- Familiar with residential settings
- Knowledge of community social service resources
- An Understanding of psycho-social principles, theories and methodology regarding human behavior, motivation and techniques necessary to fulfill professional responsibilities
- Ability to express ideas and recommendation clearly and concisely both orally and written
- Excellent and professional telephone manner, interpersonal skills, confidentiality
- The ability to establish a respectful relationship with the persons served to help them gain skills and confidence
