Job Summary
Coordinate all services to maximize consumer recovery and independence for individuals with mental illness by ensuring involvement in the Person Centered Planning Process and facilitating the implementation of Individual Plans of Service. Perform comprehensive, ongoing psychosocial assessments, participate in legal proceedings including petitioning and testifying in clinical emergencies, and provide community-based services. Facilitate coordination of consumer intakes, discharges, and transfers, communicate with all service providers in Treatment Plans, and attend hospital meetings to coordinate with hospital social workers. Monitor activities such as employment, school, volunteerism, and family caretaking while providing referrals to appropriate community resources. Work weekdays and weekends, including every other Saturday and Sunday, on the 2nd shift from 4 p.m. to 12 a.m. until medication trained, then moving to the 3 p.m. to 11 p.m. slot.
Required Qualifications
- Bachelor's degree in a human services field from an accredited institution
- Valid Michigan Driver's License
- Willingness to work in the community and perform home visits
Desired Qualifications
- LLBSW, LBSW, LLMSW, LMSW, LMFT
- Prior case management experience in a mental health or rehabilitation setting
