Job Summary
Manage a caseload of clients by monitoring their progress toward goals in service plans and ensuring participation in Ready, Willing & Able program components. Research and coordinate access to outside referral sources for mental health, substance recovery, benefits, entitlements, and primary care services. Communicate with DHS and other agencies, collaborate with clients on goal setting, and facilitate groups on various topics. Maintain accurate documentation in TDF's Client Tracking Database and DHS's CARES system while providing crisis intervention to resolve client issues. This role supports the mission to break cycles of homelessness, addiction, and recidivism at the Porter Avenue Facility.
Required Qualifications
- High School Diploma
- At least 2+ years of experience in Human Services/Case Management
- Strong interpersonal and written/verbal communication skills
- Working knowledge of MS, Word, Excel, PowerPoint, and Google
Desired Qualifications
- Bachelor's degree with a major in Psychology, Sociology, Counseling, Social Work, or other Human Services concentration
- Experience working with challenging high-service needs populations
- CASAC
- Bilingual
