Job Summary
Rapid Rehousing Case Manager provides case management and rental assistance to homeless individuals, families, and youth in Marion and Polk Counties. Responsibilities include eligibility screening, program enrollment, participant follow-ups and check-ins, processing required paperwork, data entry and general program supports. Direct services include case management and housing placement support, development of Individual Service Plans, monthly client home visits, maintaining confidentiality, and collaboration with community partners to ensure access to housing resources and compliance with funding requirements. The role aligns with Housing First initiatives and requires day-shift hours (8am-4:30pm, Monday–Friday).
Required Qualifications
- A High School Diploma or GED
- two years of related social service experience
- Driver qualifications required (valid driver's license, insured vehicle, acceptable driving record)
- pass background screenings (MWVCAA)
- pass pre-employment and random drug/alcohol screenings
- excellent planning, organization, and time management
- ability to communicate effectively (oral and written)
- knowledge of MS Office, databases, web tools
