Case Manager- Rochester

Case Manager position in Rochester, MN (part-time, 10-14 hours per week) responsible for providing community-based services to correctional clients post-release, performing intake/assessment, monitoring progress, maintaining records, and facilitating access to resources. Requires transportation, strong communication, confidentiality, and ability to travel; duties include monitoring apartment checks, safety, and CAM tasks, with a background in corrections or related human services preferred. Education requirement includes a high school diploma or equivalent; an Associate’s Degree is preferred. 10-14 hours weekly, Monday–Friday, with duties spanning verification of compliance, risk assessment, case planning, and community referrals.
