Job Summary
Coordinate waiver-based case management for individuals with intellectual/developmental disabilities; develop and submit Individual Program Plans (IPP), chair interdisciplinary treatment team (IDT) meetings, coordinate APS Medicaid and waiver budgets, ensure prior authorizations and regulatory compliance, and maintain timely documentation and confidentiality; requires a Bachelor's degree in a human services field (or RN with WV license) and experience in the I/DD field; includes ongoing required trainings and driver’s license.
Required Qualifications
- Bachelor's degree in a human services field (social work, special education, psychology or counseling) or RN with valid WV license and one or more years' experience in the I/DD field
- Bachelor's degree in a human services field and less than one (1) year experience in the I/DD field
- Bachelor's degree in a non-human service field and one year of experience in the I/DD field
- Must possess a valid driver's license
- Must satisfy criminal background requirements and have satisfactory motor vehicle report
