Case Manager RN (Temporary, Day shift)

Job Summary
Formulate a plan of care to meet identified patient needs at initial assessment and document it in medical records. Perform utilization review within 24 business hours of admission and document findings on internal worksheets. Interview patients and families regarding risk criteria for education needs, special discharge concerns, and follow-through on discharge planning. Actively participate in interdisciplinary rounds to provide staff on InterQual criteria, payor guidelines, and identified care plan variances. Identify issues requiring patient education prior to discharge. This temporary day shift role supports Hudson Regional Health's unified network of four hospitals delivering modern, patient-first care.
Required Qualifications
- Valid NJ RN
- Degree in Nursing
- Minimum 2-3 years' experience in Healthcare setting
- Influenza Vaccinations
Desired Qualifications
- BSN
- Milliman Guidelines, evidence-based clinical practice
- Successfully completes initial competency verification for specialty area
- Demonstrated people/interpersonal skills
- Ability to communicate clearly in English, both orally and in writing
- Ability to handle confidential information
