Job Summary
Develop and implement patient-specific, safe, and timely discharge plans while managing utilization reviews, resource allocation, and treatment plans. Coordinate with payors to verify utilization criteria, negotiate reimbursement, and minimize financial risk through insurance verification. Perform medical record documentation, facilitate multi-disciplinary team meetings with physicians and therapists, and ensure compliance with facility-wide utilization and quality management policies. Maintain fiscal responsibilities and social work services as needed per state guidelines. Full-time 32-hour schedule with a $5,000 sign-on bonus.
Required Qualifications
- Current licensure in a clinical discipline either as a Nurse (RN/LPN/LVN) or a Respiratory Therapist (RT) OR Medical Social Work (license per state guidelines)
- Previous discharge planning experience
- Previous RN/LPN/RT/SW/CM experience in an inpatient hospital setting dealing with critical care/acute care patients
- RN preferred
Desired Qualifications
- Specific experience in Care Management and Discharge Planning
- Working knowledge of the insurance industry and government reimbursement
