Job Summary
Coordinates, negotiates, procures, and manages the care of complex patients to facilitate quality, cost-effective outcomes; conducts admission and concurrent utilization activities, conducts discharge planning, and collaborates with an interdisciplinary health-care team to plan high-quality, realistic continuum-of-care arrangements; maintains thorough documentation, facilitates referrals, and ensures compliance with payer contracts; requires knowledge of EMR systems (Cerner), and BLS with bilingual English/Spanish preferred.
Required Qualifications
- Bachelor’s Degree in Nursing, medicine, or related field
- Certification in Case Management (CCM) or comparable certification preferred
- Current AHA Basic Life Support (BLS) certificate required
