Case Manager RN

RN Case Manager responsible for the management and coordination of the total care plan from admission to discharge. Assesses and plans care within 24 hours of admission and at least every 96 hours using Milliman criteria and medical necessity; documents and communicates with payors for authorization/reimbursement; collaborates with physicians and health-care team to develop and modify discharge plans; coordinates interdisciplinary discharge planning meetings and ensures completion of required discharge forms; identifies barriers to discharge and monitors length of stay and readmissions; works with Social Work for complex discharge needs; supports pediatric, adult, and geriatric patients; requires Florida RN license and 2–3 years of experience; educational qualifications include Bachelor's or Associate's Degree in Nursing; location Titusville, FL; full-time schedule 8:00am–4:30pm.
