Utilize an Integrated Case Management Model to follow patients through the continuum of care. Facilitate utilization review, care coordination, advocacy, and discharge planning to ensure patients receive the right care, at the right time, and in the right place. Maintain high-quality, medically necessary, evidence-based treatment for all patients regardless of payment source. Full-time day shift.
