Job Summary
Case Manager (RN, LSW, PT, OT, SLP) collaborates with the physician to manage each patient’s program through the continuum of care, coordinating clinical and financial treatment plans, and ensuring timely, cost-effective, individualized service delivery for rehabilitation patients with spinal cord injury, brain injury, stroke, amputation, neurologic/orthopedic conditions, and arthritis. Oversees length-of-stay management within CMS guidelines and maintains 60% compliance threshold; involves communication, negotiation, and ensuring appropriate reimbursement systems knowledge. Licensure required in RN/LSW/PT/OT/SLP with a minimum 3 years of hospital-based healthcare experience; preferred 1 year medical rehabilitation experience and certification in case management.
Required Qualifications
- Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, or SLP.
- Minimum of 3 years healthcare and clinical experience in a hospital setting.
