Case Manager (RN/LPN)

Job Summary
Responsible for the case management process including assessment/reassessment, service planning, and monitoring of Morton patients. Identifies high-risk patients, conducts initial screenings to determine eligibility, develops comprehensive treatment plans, coordinates delivery of services, leads Transition of Care activities, and collaborates with an interdisciplinary team to document and monitor progress. Refer social drivers of health to a Community Health Worker or designee and coordinate with care team to ensure goals are understood and met. Requires current OK nursing license (LPN or RN), 2+ years clinical healthcare experience, 1+ year in case management, BLS/CPR certification, and demonstrates ability to work with staff and the public; bilingual Spanish preferred. Safety-sensitive role with potential for post-offer medical marijuana considerations.
Required Qualifications
- Current OK nursing (LPN or RN) license in good standing
- 2+ years of demonstrated clinical experience in healthcare
- 1+ years of demonstrated Clinical Case Management or Care Management experience
- Bilingual in Spanish preferred, but not required
- Current BLS/CPR certification required
- Demonstrated ability to work effectively with staff and the public
- This position is safety-sensitive; may be subject to post-offer drug testing
