Job Summary
Conduct collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy for comprehensive health needs using available resources to promote quality, cost-effective outcomes. Manage Medicare members with diabetes, CHF, CKD, or cardiac conditions through discharge planning and home health care coordination. Communicate with individuals and families to facilitate options and services while occasionally traveling within a designated geographic area for in-person activities. Utilize proficiency with standard corporate software and proprietary applications to navigate multiple systems efficiently. Maintain independent work from home capability while obtaining multi-state RN licenses as needed.
Required Qualifications
- Compact/New York or Massachusetts License Required
- Minimum 3-5 years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
- Minimum 2-3 years CM, discharge planning and/or home health care coordination experience
- Ability to occasionally travel within a designated geographic area for in-person case management activities as directed by Leadership and/or as business needs arise
- Registered Nurse with active state license in good standing within the region where job duties are performed
- Bachelor's degree or equivalent experience, applicant would be required to obtain a bachelor's degree within 3-5 years as part of role development, state licensing laws may apply
- Will be able to obtain multi state RN licenses if needed, company will provide

- Effective communications, organizational, and interpersonal skills
- Ability to work independently (may require working from home)
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications
- Efficient and Effective computer skills including navigating multiple systems and keyboarding
- Certified Case Manager is preferred
- Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) is preferred, but not required
