Job Summary
Case Manager RN at TriStar Centennial Medical Center coordinates multidisciplinary care across the patient journey, develops individualized care plans, conducts psychosocial assessments, and facilitates education for patients, families, and caregivers to support treatment planning, discharge planning, and smooth transitions of care. Collaborates with Rehab Program Director and interdisciplinary team to ensure patient-centered care, upholds the organization’s values, and communicates patient rights and insurance/Medicare benefits throughout the care continuum.
Required Qualifications
- RN (Registered Nurse)
- Associate Degree or Bachelor’s Degree or RN Diploma
