Job Summary
Case Manager RN role in the Rehabilitation program at TriStar Centennial Medical Center involves guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare/insurance benefits, discharge criteria, and patient rights; coordinating education for patients, families, and caregivers; developing and implementing individualized treatment plans reflecting patient strengths and recovery goals; completing psychosocial assessments and discharge planning evaluations with clear documentation and communication across the care team; collaborating with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care; promoting compassion, respect, and excellence in every interaction.
Required Qualifications
- (RN) Registered Nurse
- Associate Degree, or Bachelors Degree, or Registered Nurse Diploma
