Job Summary
Case Manager RN coordinates services and develops individualized care plans for patients and families, guiding program orientation, discharge criteria, and patient rights; coordinates education for patients, families, and caregivers; documents psychosocial assessments and discharge planning; collaborates with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care; emphasizes compassion, respect, and excellence in patient and family interactions; supports safe transitions and consistent quality across the continuum of care. Requires RN licensing and either an associate degree, BSN, or RN diploma; role is full-time with no travel; benefits package offered by TriStar Centennial Medical Center.
