Case Manager - RN (For Medicaid)

Job Summary

Evaluate Medicaid client needs, build personalized care plans, and coordinate home and community-based services. Participate in daily discharge planning meetings to address barriers and target successful transitions between care levels. Create a seamless line of communication between the inpatient care management team and outpatient case management team. Work effectively under pressure to multitask and perform numerous tasks simultaneously with minimal errors. Maintain collaborative relationships throughout the Singing River Health System and other organizations while adhering to system procedures, accreditation standards, and governing guidance. Shift: Monday - Friday (8:00 AM - 5:00 PM) with some travel required.

Required Qualifications

  • Graduate of an accredited School of Nursing
  • Bachelor's Degree in Nursing
  • Currently licensed to practice as a Registered Nurse in the state of Mississippi
  • Two (2) years' nursing experience in ambulatory or acute care that has relevance to the practice of Case Management
  • BCLS certification
  • De-escalation training completed by the end of position orientation (90 days)
  • Appropriate level of de-escalation training

Desired Qualifications

  • Bachelor's Degree in Nursing preferred
  • TB certification preferred
