Job Summary
Integrated Care Management (ICM) Care Manager role for CVS Health/Aetna in Passaic/Sussex Counties, NJ. Frontline advocate responsible for assessing, planning, implementing, and coordinating all case management activities to evaluate members' medical needs and facilitate overall wellness. Conducts assessments from multiple data sources (claims, etc.), applies clinical judgment to reduce risk factors and barriers, and develops proactive care plans. Collaborates with supervisors and interdisciplinary teams; utilizes motivational interviewing to engage members. Position involves 25-50% travel within a 50-mile radius from home; work-from-home flexibility with travel. Requires active RN license in NJ, Bachelor/Associate-level education (Associate’s in Nursing preferred per posting), and 3+ years of clinical practice experience. Preferred CCM certification and home health care coordination experience. Bilingual abilities preferred. Office locations include Passaic, Jersey City, Bayonne, Union City, and North Bergen, NJ. The role emphasizes patient advocacy, coordination with providers and community resources, and ensuring adherence to regulatory and company policies. Hours are typically Monday–Friday 8-5, with compensation in the listed pay range and potential bonus eligibility.
Required Qualifications
- Minimum 3+ years of clinical practice experience
- Active and unrestricted RN license in the state of NJ
- Willing and able to travel 25-50% of their time using your own vehicle to meet members face to face in their assigned area
- Reliable transportation required
- Must reside in Passaic or Sussex County, NJ
- Associate's Degree in Nursing (Education)
- Certified Case Manager is preferred
- Minimum 2+ years Care Management, Discharge Planning and/or Home Health Care Coordination experience preferred
- Bilingual preferred
