Job Summary
Conduct case management activities for dual-eligible Medicare and Medicaid members by assessing needs, developing proactive care plans, and coordinating services with providers and community organizations. Utilize clinical tools and data reviews to evaluate medical conditions, reduce risk factors, and address complex health and social determinants. Collaborate with interdisciplinary teams at case conferences to overcome barriers and ensure compliance with regulatory policies. Apply motivational interviewing skills to engage members and facilitate their overall wellness. Travel 25-50% of the time from your Ocean County, NJ home to meet members face-to-face within a 50-mile radius using your own vehicle. Requires 3+ years of clinical RN experience and an active NJ license.
Required Qualifications
- Minimum 3+ years of clinical practice experience
- Active and unrestricted RN license in the state of NJ
- Willing and able to travel 25-50% of their time using your own vehicle to meet members face to face in their assigned area
- Reliable transportation
- Reside in Ocean or surrounding County, NJ
Desired Qualifications
- Certified Case Manager
- Minimum 2+ years Care Management, Discharge Planning and/or Home Health Care Coordination experience
- Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills
- Ability to work independently
- Effective computer skills including navigating multiple systems and keyboarding
- Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
- Bilingual
- Associate's Degree in Nursing
- Bachelor's Degree
