Job Summary
Conduct comprehensive case management for dual-eligible Medicare and Medicaid members by assessing medical needs, evaluating claims data, and developing proactive care plans to address complex health and social challenges. Collaborate with members, providers, and interdisciplinary teams to overcome barriers in meeting goals, utilizing clinical judgment and motivational interviewing to enhance short and long-term wellness outcomes. Work remotely with 25-50% travel within a 50-mile radius of your Mercer County, NJ home, coordinating referrals and presenting cases at conferences while adhering to regulatory policies. Requires active RN license in NJ, 3+ years clinical experience, and reliable personal transportation.
Required Qualifications
- Minimum 3+ years of clinical practice experience
- Active and unrestricted RN license in the state of NJ
- Willing and able to travel 25-50% of their time using your own vehicle to meet members face to face in their assigned area
- Reliable transportation
- Must reside in Mercer or surrounding County, NJ
Desired Qualifications
- Certified Case Manager
- Behavioral Health experience
- Minimum 2+ years Care Management, Discharge Planning and/or Home Health Care Coordination experience
- Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills
- Ability to work independently
- Effective computer skills including navigating multiple systems and keyboarding
- Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
- Bilingual
- Associate's Degree in Nursing
- Bachelor's Degree
