Case Manager, RN (Douglasville/Atlanta, GA)

Conduct clinical evaluations in the home of Adult Foster Care members to assess needs, develop service plans, and ensure ongoing compliance. Perform monthly visits to verify quality service delivery and address condition changes. Provide support to caregivers, review services within scope, and communicate client needs updates to the primary care physician. Develop, implement, evaluate, and revise the client's plan of care while instructing caregivers on treatment protocols. Complete all client documentation, reports, and notes accurately and timely following visits. Coordinate client care through communication and written documentation with physicians, RNs, program directors, and community resources. Interpret agency philosophy and objectives for clients and families. Maintain ethical standards, respect client and employee rights, and ensure security of protected information in accordance with agency policies.
