Case Manager RN

Nurse Case Manager coordinates and optimizes the continuum of care for assigned patients, including initial and ongoing assessments, chart reviews, case conferences, and development of individualized care plans. Acts as a liaison among primary care providers, specialists, and patients, advocates for access to services, navigates Medicaid/resources, and supports discharge planning through medication reconciliation and coordination of DME/HHC where applicable. Performs outreach and data collection to inform program operations and funding, identifies high-risk patients, and ensures adherence to clinical standards and guidelines while promoting patient safety and effective use of resources.
