Job Summary
Coordinate patient care across multiple disciplines by conducting psychosocial assessments, developing individualized care plans, and facilitating communication between patients, families, and the interdisciplinary team. Execute discharge planning evaluations, educate caregivers on treatment expectations, and identify barriers to timely transitions from admission through discharge. Document clinical findings and care plans accurately while collaborating with rehab leadership to resolve care coordination challenges. This Full-time, day shift role (8:00 AM to 4:30 PM) includes rotating weekends at a 267-bed acute care hospital in Fort Walton Beach.
Required Qualifications
- Associate degree or nursing certificate
- Active Registered Nurse (RN) license
- Minimum of 2 years of recent acute care experience within the past 3 years
- Strong communication, assessment, and care coordination skills
- Rotating weekends
Desired Qualifications
- ICU, CCU, Med-Surg, or LTAC case management experience
