Job Summary
Case Manager (RN) at Bronson LakeView Hospital coordinates patient care from admission through discharge, serving as an advanced clinical resource and coordinating with the multidisciplinary team to ensure a continuum of cost-effective, high-quality care. Responsibilities include assessment, planning, implementation, coordination, monitoring, and evaluation of patient caseload; development and revision of transition and care plans; discharge planning; and ongoing communication with patients, families, and providers. Requires active RN license in Michigan, BLS certification, and ability to prioritize multiple demands in a high-stress environment. All new hires are expected to obtain a BSN (or higher) within 8 years; Master's degree preferred. Part-time, PRN availability; typical schedule includes variable hours with wards requiring patient care and transitions management.
Required Qualifications
- Licensed Registered Nurse in good standing with the State of Michigan
- BLS certification required by completion of core orientation
- Case Management Certification preferred
- 3 years of experience in an acute care hospital setting required
- All new hires are expected to successfully obtain their BSN (or higher nursing degree) within 8 years of hire date
- Master's degree preferred
