Job Summary
Coordinate discharge plans and care transitions for a designated caseload, ensuring optimum utilization of patient and Health System resources while meeting CMS and external agency requirements. Identify patients benefiting from case management services, leading assessments, problem identification, care planning, delivery, monitoring, and evaluation specifically for heart failure patients to improve home success and reduce readmission. Manage communication among varied care providers, internal and external to the organization, and collaborate closely with the Transition of Care Pharmacist, Heart Failure Clinic, and Remote Patient Monitoring program. Full-time, M-F rotation with weekend and holiday coverage.
Required Qualifications
- Bachelor of Science in Nursing (BSN) degree from an accredited institution
- Current unrestricted licensure as a Registered Nurse in the state of Kentucky
- Three years of job-related experience
- Case Management certification (ANCC, CCM or ACM-RN) or other approved Professional Certification required within two years of hire date
