Job Summary
Case Manager I will work collaboratively alongside case managers, supervisors, integrated health team members, and community-based partners to promote long term solutions for homeless or formerly homeless families, children, and individuals. Responsibilities include developing and implementing care plans, coordinating services and resources, monitoring client progress, providing advocacy and support, facilitating referrals to other services, maintaining case documentation and data collection, and applying motivatational interviewing, stages of change, strengths-based case management, housing first, harm reduction, and trauma-informed care. The role focuses on delivering outreach services, facilitating referrals and linkages to mainstream services and housing, and providing crisis intervention.
Required Qualifications
- High School Diploma or GED required
- Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice preferred
- Valid driver’s license required
