Job Summary
Provide case management services in community and office settings by assisting patients and care teams in planning, connecting, outreaching, advocating, and monitoring needs and progress. Facilitate access to health services, financial assistance, employment, education, and social supports while assessing patient needs and developing individualized treatment plans. Document services provided accurately and timely, adhering to CBI Clinical Policies and Procedures. Requires high school diploma, six months of recovery experience, and 1-3 years working with the GMH/SU population. Hourly pay with generous PTO, medical/dental/vision, 401K match, and tuition reimbursement.
Required Qualifications
- Highschool diploma or GED
- 1-3 years of experience working with the GMH/SU population
- 1-3 years of experience in behavioral health position providing outreach and engagement activities
- Valid identification
- Current AZ Driver's License (valid and in good standing)
- 39-month MVR
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
- Arizona Fingerprint Clearance card
Desired Qualifications
- Associates Degree or higher in a field related to behavioral health
- Minimum of six month of recovery from substance use and/or mental health disorders
- 1-3 years of experience in a medical setting
- Current AZ Driver's License (valid and in good standing) [preferred]
- 39-month MVR [preferred]
