Case Manager

Job Summary
Determine resources needed to meet the individualized needs of participants and serve as an advocate to enable them to receive necessary services. Act as part of a multidisciplinary team to develop recovery plans, review progress, and maintain relationships with community agencies to connect participants with available resources. Provide community-based services focusing on education, resource connection, and achieving recovery goals, including driving participants to appointments within the service delivery area. Complete all assessments and documentation required by RHD, funders, and licensing entities. Work Tuesday through Saturday from 9am to 5pm reporting to the Program Director.
Required Qualifications
- Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services
- Valid driver's license
- Must be available for Tuesday - Saturday 9am - 5pm shifts
- Must be located in Haddon Heights, NJ
- Must be able to drive program participants to appointments
- Must be able to lift 50 lbs
Desired Qualifications
- Bachelor's Degree in Human Services or related field
- One year Case Management experience
- Intermediate computer skills
