Job Summary
Case Manager – Residential: carries caseload, meets with clients weekly, links clients to providers, escorts to psychiatric/medical care, ensures safe, client-centered care, prioritizes safety in crises, organizes and escorts activities, tracks participation, maintains audit-ready client records, upholds confidentiality, and performs other duties as assigned to support homeless and health-social service clients.
Required Qualifications
- Bachelor’s Degree (Social Services field preferred)
- Minimum of 2 years in human services
- Crisis intervention skills
- Familiarity with Health Insurance Portability and Accountability Act (HIPAA)
- Willing to participate in continuing education
