Job Summary
Conduct assessments, develop and adhere to case plans, and facilitate staffing meetings for assigned clients across various Residential facilities. Complete case notes, quarterly reports, and staffing reports following standard Case Management practices. Maintain a valid Wisconsin driver's license and insurance while working on an as needed basis within our residential programming.
Required Qualifications
- Bachelor's degree or Associates Degree in social service field
- Valid WI driver's license
- Insurance

- One to one year of related experience
