Case Manager, Resident Support

Job Summary

Conduct brief assessments, information referrals, and service linkages for assigned residents to maintain housing placement and enhance self-sufficiency. Complete Individualized Service Plans (ISP) and establish strategies to address problem behaviors or needs. Provide time-limited follow-up support as clients access new services and initiate action plans. Maintain up-to-date case notes, telephone contact logs, and referrals by entering client data into required databases daily. Participate in team meetings, service planning discussions, and engagement team meetings to coordinate service delivery with Property Managers and management. Ensure operational readiness for new properties and submit necessary funder reports in a timely manner. Work under supervision of the Support Services Manager, adhering to SMOC policies and funder requirements. On-call and after-hour coverage will be required. Monday through Friday, 3:00 PM to 11:00 PM, 35 hours per week.

Required Qualifications

  • Bachelor's Degree
  • CDAC
  • 3 years experience in related field
  • one year assessment, advocacy, and case management skills
  • Sensitivity to low-income people of diverse backgrounds
  • Excellent listening skills
  • Ability to work independently
  • Good written and verbal communication skills
  • valid driver's license
  • reliable transportation
  • meet insurance standards
  • Must be able to stand for extended periods of time
  • Must be able to lift 50 pounds
  • A successful background check
