Job Summary
Provide case management and service-planning for adults with traumatic brain injury and related disabilities in home/community and office settings serving Nassau and Suffolk counties. Develop individual service plans, coordinate linkages to outside resources, and collaborate with personnel per Department of Health qualifications. Requires an Associate degree with 2+ years of experience; positions are part-time with day-shift hours (Monday–Friday) and a flexible schedule; work includes home/community-based and office duties; driver’s license required.
