Case Manager - Rehabilitation Management 1

Job Summary
Maintain contact with claimants at their home, hospital, or workplace to assess situations and conduct cost-benefit analysis for developing rehabilitation and return-to-work plans. Facilitate workplace assessments, accommodations, and medical consultations while gathering medical information to determine entitlements. Collaborate with Health Care Services, senior case managers, and community partners to promote quality, cost-effective interventions and outcomes. Requires a degree in Psychology, Social Work, or Rehabilitation; three years of case management experience including one year in insurance customer service. Must be willing to travel within Manitoba, have access to a personal vehicle, and hold a valid driver's licence.
Required Qualifications
- Degree in Psychology, Social Work, Medical Rehabilitation (Kinesiology or Athletic, Occupational, or Physical Therapy) or a related discipline
- Degree in Business, Arts or related discipline
- Certificate in Rehabilitation Case Management, Rehabilitation Benefit Administration, Disability Management Practitioner, or related case management discipline
- Three years of case management experience
- One year experience in customer service in insurance
- Ability to identify, analyze, research, recommend, and implement effective solutions to a variety of situations
- Ability to work effectively as part of a team
- Ability to communicate effectively, orally and in writing, to clearly convey complex information in a concise and straightforward manner
- Ability to work with a high levels of initiative
- Willingness and ability to travel within Manitoba
- Access to a personal vehicle
- Valid driver's licence
