Job Summary
Case Manager responsible for coordinating rehabilitation patient care and guiding progression through the continuum of care. Conducts initial patient assessment within 72 hours, develops tentative discharge plans with patient and family goals, and collaborates with the interdisciplinary team and payers to ensure timely, cost-effective, individualized services. Maintains up-to-date documentation in medical records, coordinates team conferences and discharge planning, communicates progress to family and external payers, and supports utilization management and program evaluation. Requires Louisiana RN licensure and knowledge of CMS guidelines and related workflows.
Required Qualifications
- Current Louisiana Licensure as RN
- Fifth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience
- Knowledge of basic bookkeeping, office procedures, and electronic billing procedures
- BCLS is required
