Job Summary
RN Case Manager coordinates the patient’s care and services in a post-acute setting, promoting quality care through collaboration with patients, families, the care team, payers, and external entities to ensure safe transitions between facilities and care settings. Requires an active RN license and nursing degree (associate’s or bachelor’s). Prefer CCM or RN-BC board certification and one year of clinical post-acute experience. Responsibilities include generating clear written communications, prioritizing and organizing tasks, coordinating care, data-entry, and serving as a liaison among physicians, family, and community resources; ensuring adherence to policies, standards of practice, and Life Care’s Code of Conduct.
Required Qualifications
- Nursing diploma (associate’s or bachelor’s degree in nursing)
- Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
