Case Manager Registered Nurse

Job Summary
As a Case Manager Registered Nurse, provide case management for inpatient and observation patients; coordinate services, develop care plans, and drive safe transitions with physicians and multidisciplinary teams; perform comprehensive psychosocial, medical, and discharge needs assessments; collaborate with Social Services to ensure appropriate post-acute care; use InterQual criteria to evaluate progression of care; act as a liaison among physicians, patient/family, hospital staff, and outside agencies; support patient-centered care and discharge planning to optimize outcomes.
Required Qualifications
- 2+ years experience in Case Management OR 3+ years experience in a clinical setting
- Associates Degree in Nursing
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation
- BLS
