Job Summary
Coordinate and manage patient care plans across healthcare settings by assessing needs using clinical data, history, and diagnostics. Collaborate with providers to support safe transitions from hospital to home while conducting utilization reviews for cost-effective care. Track patient progress, manage disease workflows, and leverage digital tools to monitor performance. Connect patients with community resources to enhance care outcomes. Requires an active RN license, 3+ years of acute care experience, and an Associate Degree in Nursing. Working hours are 8:00 am to 4:30 pm.
Required Qualifications
- Active & unencumbered RN license (required) in the state of residence
- 3+ years of acute care clinical experience as an RN
- Associates Degree in Nursing required
- Strong problem-solving and decision-making skills
- Knowledge of medical terminology and clinical workflows
- Comfort with digital tools and healthcare systems
- Ability to communicate clearly and handle sensitive information with care
- Passion for patient advocacy and working with diverse populations
- Working scheduled is 8:00 am to 4:30 pm in your time zone
Desired Qualifications
- A compact RN license is preferred
- Managed care experience preferred
- Home Care experience preferred
- Certified Case Manager (CCM) preferred
- BSN preferred
