Job Summary
Conduct telephonic and occasional face-to-face assessments to evaluate member healthcare needs and develop individualized care plans that improve health outcomes. Coordinate services and resources to support members with complex medical and social needs while applying clinical expertise to identify risks and facilitate successful transitions. Partner with multidisciplinary teams to optimize member outcomes and support effective claim management using motivational interviewing techniques. Requires 3+ years of acute care hospital RN experience with 3–5 years in oncology, an active unrestricted RN license, and an Associate Degree in Nursing. Monday–Friday schedule with flexible 8-hour shifts between 8:00 AM–6:00 PM, occasional late shifts, and less than 5% travel.
Required Qualifications
- Active, unrestricted RN license in state of residence
- Willingness to obtain additional state licenses as needed
- 3+ years of acute care hospital RN experience
- 3–5-year acute oncology experience
- Associate Degree in Nursing (ADN)
- Monday–Friday schedule with no weekends or holidays required
- Flexible 8-hour shifts between 8:00 AM–6:00 PM in your local time zone
- Occasional late-shift rotation until 8:00 PM
- Less than 5% travel for potential clinical audits
Desired Qualifications
- Compact RN license
- Certified oncology nurse or certified chemotherapy nurse
- 1+ year of Case Management and/or Home Health experience
- Certified Case Manager (CCM) certification
- Proficiency with Microsoft Office (Word, Excel, PowerPoint)
- Excellent telephonic communication and member engagement skills
- BSN
