Job Summary
Conduct comprehensive telephonic assessments and develop individualized care plans for members with complex healthcare needs. Identify health risks, coordinate referrals, and collaborate with multidisciplinary teams to improve member outcomes while maintaining regulatory compliance. Apply clinical judgment to support members with co-morbid conditions and navigate resources effectively. Monday–Friday availability between 7:00 AM – 6:00 PM CST with flexible scheduling options. Requires an active unrestricted RN license, minimum 5 years clinical experience, and an Associate Degree in Nursing. This role supports CVS Health's mission to simplify healthcare and provide a compassionate, connected health experience.
Required Qualifications
- Active, unrestricted RN license in state of residence
- Willingness to obtain a Compact RN license (company-paid)
- Minimum 5 years of clinical RN experience
- Associate Degree in Nursing (ADN)
- Ability to work within Monday–Friday business hours
Desired Qualifications
- 6+ months of Case Management or Utilization Management experience
- Case Management Certification (CCM or equivalent)
- BSN
