Job Summary
Case Manager RN opportunity at Parkridge Medical Center. Under the supervision of the Nurse Manager, the RN Case Manager performs the primary function of an RN in assessing, planning, implementing and evaluating patient care. Responsibilities include problem solving, evaluating data and preparing reports with recommendations, maintaining confidentiality per hospital rules (HIPAA), participating in continuous quality improvement activities related to quality control/assurance, safety and inspections, and contributing to the department’s mission through teamwork and support of colleagues.
Required Qualifications
- Graduate of an Accredited RN program
- Registered Nurse (RN) license required or RN Compact State Licensure in the State of Residence
- Case Management Certification (CCM) or American Case Management Association (ACMA) certification preferred
- 3-5 years acute care experience and Case Management or Utilization Management preferred
