Job Summary
The Case Manager—Registered Nurse (CM RN) coordinates comprehensive care for SNP members with chronic medical and behavioral health conditions, addressing social determinants of health (SDoH) needs. Responsibilities include telephonic member engagement (50-75% of time), developing individualized care plans with goals and interventions, providing disease management education, ensuring interdisciplinary team involvement, connecting members to health plan benefits and community resources, meticulous documentation in the electronic health record, and participating in process improvements and mentoring. Requires an active RN license, ASN degree, 3+ years nursing experience, 2+ years of case management/home health coordination, and proficiency with Microsoft Office; bilingual English-Spanish and Medicare/Medicaid care management experience are preferred.
Required Qualifications
- Active and unrestricted Compact Registered Nurse (RN) licensure in the state of residence
- Education: Associate's Degree in Nursing (ASN) REQUIRED
- 3+ years of nursing experience
- 2+ years of case management, discharge planning and/or home healthcare coordination experience
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams)
- Ability to work from home with private space
- Strong communication and collaboration skills
- Experience with Medicare/Medicaid managed care (preferred)
- Bilingual English-Spanish (preferred)
