Job Summary
Case Manager RN position at CVS Health focused on facilitating benefits/healthcare information to determine eligibility, promoting wellness, and coordinating care. Required Qualifications
- Active Compact License
- 3-5+ years clinical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
- 2-3+ years CM, discharge planning and/or home health care coordination experience
- Ability to occasionally travel within a designated geographic area for in-person case management activities
- Education: Associates Degree Required
- Bachelors Degree Preferred
- 40 hours weekly
- RN license; willingness to obtain multi-state licenses if needed
