Job Summary
The Reentry & Recovery Case Manager provides professional, wrap-around case management services to participants in the Reentry & Recovery program, including strengths/needs assessments, development of person-centered case plans with goals, linking and coordinating services, planning for successful community reentry with employment and housing, and ongoing collaboration with Probation/Parole. The role involves maintaining case records, coordinating with interagency partners, assisting participants with access to community resources, and mentoring Peer Support Specialists; responsibilities include safeguarding privacy, supporting program staff, and contributing to referrals from multiple departments.
Required Qualifications
- Associate Degree in Human Services or related field
- Three (3) years related experience
- One (1) year experience working in reentry/recovery or a closely related field
- Knowledge of Peer Support and willingness to be trained for certification
- CPR/First Aid/NARCAN trained or willingness to be certified soon after date of hire
- Must be 21 years of age with a Valid Driver’s License
