Case Manager/Qualified Behavioral Health Specialist

Job Summary
Coordinate assessments, treatment planning, and crisis intervention services for children, adolescents, and adults with mental health conditions. Facilitate healthcare and behavioral health services as part of care coordination, while providing training and linking clients to community resources. Monitor symptoms and service delivery to ensure client needs are met, and assist in obtaining necessary services for basic necessities. Provide interventions to aid the restoration of daily living skills and coordinate crisis management and stabilization. Collaborate effectively with treatment teams and families to foster recovery through individualized support and social network expansion. Requires a valid driver's license, willingness to travel throughout the assigned service area, and three years of direct experience.
Required Qualifications
- High School diploma
- 3 years of direct experience
- Valid driver's license
- Car insurance
- Insurable driving record
- Willingness to travel throughout the assigned service area
- Must have reliable transportation
- An understanding of the community mental health system
Desired Qualifications
- Associates or Bachelor's degree in a social sciences field or related field
- An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior
- An understanding of how to therapeutically engage a person with mental illness
